The BONE LAYER technique is indicated for the regeneration of large buccal atrophic ridges, usually in partially edentulous patients.
It involves the placement of a subperiosteal lamina and allows horizontal and vertical regeneration of significant volumes.
Depending on the site, the technique requires release incisions in the soft tissues to increase the availability of soft tissue covering the newly generated bone volumes.
Beneath the lamina, a collagenated xenogeneic bone graft is required. The average re-entry time is expected to be 6 months, during which the lamina integrates but is not replaced, while the xenograft is generally almost completely replaced. Complete replacement of the lamina takes approximately 12 months.
A collagen membrane must be placed over the lamina to isolate the soft tissues from the graft not covered by the lamina (the lamina acts as the vertical buccal wall, but not the occlusal one).
Unlike the use of meshes, even customized ones, the lamina does not need to be removed, which allows flapless implant placement 4 to 6 months after the first surgery.
